We are currently looking for applicants to interested in an exciting vacancy for an experienced Recovery Practitioner. The successful post holder will be responsible for providing primary care support within GP surgeries across the Three Boroughs
(City of Westminster, Royal Borough of Kensington and Chelsea and Hammersmith & Fulham). The key purpose of the role will be to provide psycho-social interventions alongside Opiate substitute treatment provided by the GP.
This post will form part of a multi-disciplinary team providing an integrated service with GPs
delivering the prescribing element within a GP Shared Care model across the Three Boroughs.
The post holder will carry a caseload of patients on the GP shared care scheme, contributing to all
aspects of care including assessment, titration, care planning, EMIS data inputting and TOP
The post holder will also deliver targeted drug information/advice and campaigns in line with service
needs and developmental plans.Responsibilities will include:Managing your own caseload of Service UsersAssisting service users wanting to go through the referral process and conducting assessments for suitabilityWorking as part of a multi-disciplinary team with a holistic approach to needs of service usersSupport and advise healthcare professionals on opiate substitute treatmentCare planningProviding support and advice on harm minimisationOperation of the intermediate care clinic (a satellite prescribing clinicPromote recovery across the three boroughs GPs and pharmaciesAttending GP meetings and clinic review meetingsDelivery of targeted drug information and advice campaigns across the areaAccurate input and monitoring of service user data using the case management systemWork in accordance with relevant governance and internal policies and proceduresFor more information and a full job description, please use the apply links provided, or send a CV to #####@######.### or call us on ##### ######RequirementsMinimum of 2 years experience in a similar roleLevel 3 diploma in health and social careOR - an appropriate professional qualification plus level 3 working with substance misuse Experience of working in substance misuse and harm reduction 1-3 years
