About the job

Randstad Care is currently recruiting for an exciting new role as a social worker to join a hospital discharge team based in Manchester. This contract will run for 6 months with the possibility of extension.

The successful candidate will be responsible for:
Working in an acute General Hospital discharge setting
Directly working with service users and carers to assess needs, assets and risks to independence
Working independently, while planning & prioritising caseloads
Developing, monitoring and reviewing independence / support plans that reflect service user and carer choice and control, promote and maximise independence, social inclusion and personal responsibility, and continue to meet eligible needs
In order to be considered for this role, you must:
Good knowledge of the Care Act 2014 and strengthbased assessment processes
Have minimum 2 Years experience within a Hospital discharge setting
A Degree/Diploma in Social work
