Community Integrated Care is currently seeking caring and committed people to support individuals with learning disabilities and mental health needs to live their lives to the full. With a positive, outgoing, and energetic nature, you will provide a highly individualised service, respecting the needs and wishes of people in their own homes.
This is an exciting opportunity to work in a brand new, bespoke service that has been developed in partnership with East Sussex Council to support individuals in their own supported tenancies. Individuals will have tailored care and support plans using a Positive Behavioural Support Model to ensure that they enhance their lives and opportunities.
Individuals will be following a professionally constructed support plan following discharge from hospital. They will require support within their homes, to access the community and to manage anxiety and challenges that they face due to their diagnosis.
Putting the preferences of this individual at the heart of everything you do, you will assist individuals to become more independent. You will be comfortable supporting them with all aspects of daily living, help with meals and potentially intimate care tasks. You will also provide social support and promote his independence so that he can become more active and achieve goals and aspirations.

An upbeat and positive manner is essential, as is having a genuine passion for supporting people to achieve their potential.
You will also hold an NVQ Level 3 Health and Social Care or be willing to work towards this within an agreed timescale.
Additionally, you will be required to undertake PBS Practitioner awareness training and will be able to coach and mentor Support Workers who you will work alongside. This role requires you to lead the shift and de-brief with colleagues following incidents of behaviours that may challenge.
Experience in working using a PBS approach would be a strong advantage.
