About the team:
Adolescent Outreach Team (AOT) is a highly experienced and dedicated multidisciplinary team that provides an acute Tier 4 community service to high risk 12 to 18 year olds experiencing a range of mental health difficulties. We work predominantly within the home from our base in Springfield Hospital, aiming to prevent hospital admissions, and also provide support to inpatient teams to enhance discharge packages and support early discharge.
About the role:
The post-holder will work within AOT as a Case Worker providing Care Coordination and Assessment of an agreed caseload of AOT clients.
The post-holder must be committed to effective team working and to the coherent delivery of the care this will include individual, family, group work and telephone support

In this role you will be able to work in a service currently operating a service 8am till 8pm Monday to Friday, and 11am till 7pm on weekends and bank holidays. The shifts will include 8am-4pm, 9-5pm and 12pm-8pm on weekdays and 11am-7pm on weekends, giving you the very best of good work life balance.

Our Trust headquarters is located within the impressive grounds of Springfield University Hospital in Tooting and we also operate in nearly 100 other locations throughout the UK.
