*Care Coordinator*

A brand-new opportunity has arisen for a Care Coordinator to join our busy Extra Care development for the over 55’s in Stratford & Southam. As part of this role you will be responsible for schedule the care hours for 3 developments across Warwickshire, roughly around 800 hours of care per week. Ideally you will have your NVQ level 3 in Health & Social Care or working towards this.

Unique Senior Care provides outstanding home care services, supporting predominately the elderly. We offer completely bespoke packages of personalised companionship and care to the elderly to help them maintain their independence and continue to live in their own homes.

*Responsibilities*

· The Care Coordinator will ensure that all regulatory and contractual standards are met and exceeded.

· The Care Coordinator will provide leadership and management to an assigned team of caregivers. They will ensure all teams have the skills and competences required and will provide a focus on continuous professional development within their team.

· They will be responsible for ensuring all current clients are properly assessed and that high quality, person-centred care is planned for and provided.

· The Care Coordinator will ensure that Unique Senior Care is appropriately represented and working in effective partnership with key local service providers, including local NHS partnerships, Social Work departments and community contacts.

. The Care Coordinator will act as an ambassador for the organisation.

· Communicate effectively with caregivers, clients, families, and other parties.

· Manage caregiver schedules including planning and manging Annual Leave and other requests for time off

· Manage all client schedules, ensuring care plan outcomes and requests are being met

· Match Caregivers and Clients to ensure that our clients are supported by appropriately skilled staff

· Maintain accurate client and Caregiver records on our people management system, ensuring all discussions our recorded in the relevant notes section

· Maintain compliance with all legal and company requirements at all times

*What’s on offer*

· £20,602 per annum

. 9am – 4pm, alternate weekends and some on call cover

· 35 hour contract
