Integra has 8 registered care homes across South and West Wales in Carmarthen, Llanelli, Swansea, RCT and Cardiff. The role will be to cover or services across these areas. To play a pivotal role in the referrals process including assessments and completion of relevant paperwork. Attendance at MHRT and other statutory meetings. Support managers and key staff in both developing and maintaining an understanding of the ‘best practice’ principles of relapse prevention, promotion of mental health and risk management. To provide clinical supervision for nurses across NCG Developing home manager’s competence and ability in respect of promoting Integra’s recovery framework. To review and update systems and processes relating to clinical risk management. Management of critical risks in relation to compliance with treatment and care, relapse prevention and mental health. To support the production of high quality 117/CTP and reports. Supporting the managers and key workers to conduct reviews of care plans, client support packs, daily routine and planning. Support the completion of referral assessments and make recommendations to the admissions panel. Lead and deliver on clinical practice at Team Learning Forums Provide support and guidance to develop the competencies of care practitioners. Support staff to facilitate environments and develop a culture that promotes recovery. Participate in the on-call rota providing 24-hour support across the organisation. The Ideal Candidate will have Experience supporting adults with mental health conditions. HCPC registered. Ability to work as part of a team and one Driver and willing to work across our services.
