We are currently looking for a Community Learning Disability Nurse (Epilepsy Service) on behalf of our Client, Hertfordshire County Council.
Are you a qualified RNLD with an advanced post grad qualification in Epilepsy?
The main areas of responsibility will be :
To fulfil the role of a senior community nurse within the Epilepsy Specialist Nursing Service.
To take an active role in the day to day service delivery of the ESN service and ensure that a high quality, effective and focussed epilepsy service is delivered to people with a learning disability in Hertfordshire.
To provide advice and support to health professionals, primary, secondary and tertiary services, health and social care, community services, education services and private and voluntary sectors.
To contribute at a local level to the implementation of service strategy and policy development.
This post has a county wide remit and will include the management of complex epilepsy caseloads.
Clinical and Operational
To carry a caseload and operate as an allocated nurse to service users referred to the epilepsy specialist nursing service.
To undertake detailed health & risk assessments on allocation of new referrals as identified in weekly referrals meeting.
To establish an appropriate specialist course of responsibilities/actions through individual epilepsy care plans for service users including provision of rescue medication guidelines.
To adhere to Care Programme Approach (CPA) policy.
To carry out joint assessments with other Multi-Disciplinary Team (MDT) members.
To give clinical and up to date advice to service users and carers regarding medication, and to liaise with pharmacy according to the individuals needs and abilities.
To organise and actively participate in the project work of the team as necessary (eg special interest groups, seminars).
To liaise with wards and departments within the acute hospitals, as appropriate - actively promoting inclusion, via a health facilitation role.
To liaise with primary health care professionals, educating and promoting the needs of individuals with learning disabilities.
To make reactive recommendations as necessary when working alone within the community.
To maintain and build upon expert levels of clinical competence.
To manage own caseload of complex cases and maintain appropriate documentation/records that conforms to the Nursing and Midwifery Council ( NMC) guidelinesTHE IMPORTANT BIT
