Job Title: Community Connector Location: Long Stratton, Norfolk Salary: £18,746 £21,268 per annum Job Type: Full Time 37 hours per week: temporary 31st March 2020 Closing date: 05/03/2018 Job Summary The role of the Community Connector is to support the provision, coordination and development of Social Prescribing; supporting individuals and families to access support within their local community from community, voluntary and publicsector organisations. The Community Connector will contribute to the wider early help and prevention approach, by supporting the development of strong, resilient and sustainable communities. What you would be part of: Due to the expansion of their Community Connector scheme we are looking for enthusiastic and selfmotivated individuals to take on this rewarding role. They are looking for Community Connectors to join their existing team working to bridge the gap between residents, community groups, the voluntary sector and statutory agencies. The objective is to encourage local people of all ages to identify and access support earlier than they currently might. Main duties & responsibilities: You will offer support and advice, gain the trust and confidence of families and provide practical assistance to ensure people can access the right local services at the earliest opportunity. Working within set geographical boundaries within the district. Provide the companies Social Prescribing service. Based for part of your week in local GPs surgeries you will be assisting people referred by health professionals with a variety of nonmedical needs who may benefit from interventions within the Early Help framework. You must have: Experience of supporting individuals and / or families on a 1:1 basis, as well as a knowledge of voluntary and community services in Norfolk and/or how to find out about local services and how to support people to access them. You should also have an ability to maintain effective working relationships and a positive attitude towards collaborative work with peers, colleagues and other professionals. Whats in it for you?
